I'm using AuthSMTP for a project that needs send-to-a-friend functionality (we're also hosting it on Engine Yard Solo / Amazon EC2 which has problems with blacklisted IP's).  Not totally sure if this is the greatest solution yet, but hopefully it solves the problem you've described.

Unfortunately, AuthSMTP isn't free, it's a paid service.

I am experiencing similar problems with gmail - mail sent from my Rails app go straight to junk - though my server is not listed on the SBL, PBL, or XBL.   Are you still having problems getting mail through to gmail, or did it all get cleared up for you?   I'm having a hard time pinpointing exactly what's flagging it, and was wondering if you'd had any more luck.
